What happens if you use it?
One random modifier is added to a rare item.
All existing modifiers and the base item remain.
The rare is already useful and has an open modifier slot.
The added modifier is random and may add little value.

An Exalted Orb adds one random modifier to a rare item that still has room for another modifier. It does not replace the item's existing modifiers or let you select the new stat. The rare item's current strengths remain, while one additional unknown result is added.
Use it only when the item has room to grow.
The item needs an available modifier slot, and its existing modifiers should be worth preserving. In Path of Exile 1, rare items can carry up to six random modifiers, so checking the current modifier count and understanding what can still be added comes before spending the orb.
The added modifier is random and can contribute little to the item. Confirm that the rare item has an open modifier slot and that you are comfortable keeping an unpredictable result. The Path of Exile 2 orb has the same short game-record description, but appears much earlier in that game's progression.
A crafting use and a market value are different questions.
An Exalted Orb has direct crafting utility, but that does not give it one permanent exchange rate. Its buying power moves with the league, supply, player crafting demand, and the currency used for comparison. Read a time-stamped market observation before treating an old ratio as current.
