A Path of Exile 2 Exalted Orb keeps every modifier already on a rare item and adds one new random modifier, provided the item has space. That makes it an expansion rather than a reset: the known part of the item stays intact, but the added part cannot be chosen.
An early way to complete a promising rare.
The imported record begins at drop level 5, so players can encounter this crafting decision early. It is easiest to understand as a way to add another chance of usefulness to a rare item whose current modifiers are already worth keeping. Check that an open modifier slot actually remains first.
The new modifier may not help the build or item at all, and the orb cannot be applied when no modifier slot remains. Do not confuse adding a random modifier with choosing a targeted upgrade, even when the rest of the rare item is already strong.
